HRT Combinations

Understanding Hormone Replacement Therapy (HRT):

Hormone replacement therapy (HRT) involves the administration of estrogen and, in some cases, progestogen to alleviate menopausal symptoms and support women's health during the transition into menopause. HRT can help relieve most, if not all, of the symptoms during menopause and will help a woman feel more like her usual self. Giving her the ability to function well during this incredibly difficult time.

Variations in HRT Brands:

​

There are numerous HRT brands available on the market, each with its own unique formulation, dosage, delivery method, and combination of hormones. These variations can influence the effectiveness, tolerability, and safety profile of HRT for individual women, making it essential to consider factors such as hormone type, dose, route of administration, and treatment duration when prescribing or selecting an HRT brand.

Estrogen Formulations:

​

HRT brands may contain different types of estrogen, including estradiol, estrone, and conjugated equine estrogens (CEEs), which can vary in potency, bioavailability, and metabolic effects. Some formulations may also include synthetic estrogens or plant-derived estrogens, such as estradiol valerate or estradiol hemihydrate, which may have different pharmacokinetic properties and metabolic pathways compared to natural estrogens.

Progestogen Combinations:

In addition to estrogen, HRT brands may contain progestogens to protect the uterine lining and reduce the risk of endometrial hyperplasia or cancer in women with an intact uterus. Progestogens can be added to estrogen therapy in various formulations, including micronized progesterone, medroxyprogesterone acetate (MPA), norethisterone, or levonorgestrel, each with its own unique pharmacological profile and potential side effects.

Delivery Methods:

HRT formulations are available in various delivery methods, including oral tablets, transdermal patches, topical gels or creams, vaginal rings, and intramuscular or subcutaneous injections. Each delivery method has its own advantages and disadvantages in terms of convenience, absorption kinetics, compliance, and side effect profile, allowing women to choose the option that combination that best suits their preferences and lifestyle.

Individualized Treatment Approach:

Given the diversity of HRT brands and formulations available, it's essential to adopt an individualized treatment approach that takes into account women's medical history, symptoms, preferences, and risk factors when prescribing or selecting an HRT regimen. Healthcare providers should conduct a thorough evaluation and discussion with women to assess their needs, educate them about the benefits and risks of HRT, and collaboratively develop a treatment plan tailored to their specific circumstances.

As I’ve mentioned before, HRT isn’t a magical silver bullet that will bring your partner back to you immediately. HRT is administered in low doses and then gradually increased over time until it has a positive effect. The doses usually take around two to three months for their benefit to become noticeable. Once the three months is up, it’s time to assess the effectiveness of the dosage and increase it slightly. Then it’s another three months of waiting until you can increase again. So be prepared for a very long wait. Especially if your partner has had an operation that has caused a sudden drop in hormone levels like mine did.

To make the situation even more complicated and confusing, as we’ve discussed, there are different methods that can be used to absorb the HRT medication. So it may be that your partner is offered estrogen patches with progestogen tablets, so she can take them in the evening and improve her sleep. But this may not work. That particular combination may not be effective at all. For my ex, they eventually settled on combination patches and they worked once they were at the right levels.

 

Conclusion:

Overall, it took around a year for the HRT to finally work. And for those particular levels she suddenly started feeling the effects after using the patches for two months. This can be an extremely difficult and frustrating time for you as you wait to see if she will return to normal. My advice is to be patient. Don’t expect miracles straight away, and be prepared for plenty of disappointment along the way.
